Outdoor Adventure Required Health Forms
Please fill out all of these forms WELL BEFORE Long-term camp. Some require your PCP to fill out so please plan ahead! These will need to be refreshed annually.

Camping is fun! Paperwork is no fun, but it is needed in case of an emergency, and a scout is prepared! Please turn in the following completed documents before participating in any camping trips with Troop 564. Turn these in to the Outdoor Coordinator or Scoutmaster.

- Troop 564 Permission Slip
- Colorado Addendum
- Colorado Addendum Additional Information
- Photocopy/Digital Image (both sides) of current Medical Insurance Card
- Copy of current YPT Certificate (Adult Leaders only)
